Terms and Conditions

Please see below a copy of my Client and Therapist Contract of Service which includes information concerning my fees. My fee is the same regardless of the therapy or combination of therapies chosen:

Contract for Therapy

 

This document is a contract between Kathryn Creed and her client.

Therapeutic Process

The therapeutic process may incorporate a variety of techniques including, but not limited to: Counselling, Psychotherapy, Mindfulness and Hypnotherapy. It is not possible to make any definite guarantees as to the success of any of these techniques as results are dependent on the individual client's response.

Sessions

The initial meeting provides us with an opportunity to decide whether to engage in a contract for therapy.  If it is not appropriate for us to contract together, I will endeavour to provide you with an alternative therapist or agency, should that be required.

I suggest that sessions are booked to suit your ongoing requirements and emergent needs.  If at any time you wish to terminate the arrangement, you can do so without any required period of notice.

Fees

The session fee will be £55 per houror £70 for 90 minutes, payable by BACS, Credit/Debit card or in cash either before or at the end of each session. Session fees are subject to review on an annual basis.

Confidentiality

All sessions, including online and telephone conversations, e-mail and text/SMS correspondence, will be conducted in confidence.  This confidence will be maintained and applied to any and all records in accordance with the General Data Protection Regulation (GDPR) and Data Protection Bill (2018), except in the following instances:

i.     Where the Client gives written consent for the confidence to be broken.

ii.    Where the Therapist is compelled by a Court of Law.

iii.   Where the information is of such gravity that confidentiality cannot be maintained; for example where there is a possibility of harm to self or others, in cases of fraud or crime, or where minors (under 18) are concerned.  

iv.   During Supervision, in which instance Client’s identity will be protected.

v.    Where a referring GP or agency requires a report.  A copy of the report will be available to the Client.

Where disclosure is required under the above circumstances, this will be on a ‘need to know’ basis, so that only those individuals or organisations who need to know in order to provide care to you, and for the proper administration of Government (whose personnel are covered by strict confidentiality rules) will be given the information.  Only information that the recipient needs to know will be disclosed.

 

Your Personal Information

I need to maintain accurate personal data in order to provide you with safe and appropriate care.  Your personal details and client notes are held in a secure manual filing system which is accessible only by myself.  Any digital communication (e-mail, text/SMS etc) is held on password-protected devices which only I can access.  I will retain your records for seven years after the end of your therapy, after which they will be destroyed securely.

Contact Permission

Your privacy is important.  The contact and personal details which you have provided will only be used to contact you in the context of your therapy, and not for any other purpose.

Cancellation of Sessions

For cancellation of sessions by the Client, a period of 3 working days notice for individual sessions is required, otherwise the session fee will be payable.  For cancellation of sessions by the Therapist, the client will be given reasonable notice wherever possible. 

 

Code of Ethics and Conduct

I work in accordance with the Codes of Ethics and Practice of The National Hypnotherapy Society and The National Counselling Society.

 

Use of Recordings

Any recordings or other materials, which I may provide for you to use at home, must not be used whilst driving or operating machinery but only when it is safe for you to relax completely away from distraction.  The recordings or other materials must not be reproduced or used for commercial services or financial gain. They are for your personal use only.

Supervision

Good therapeutic practice recommends the regular supervision of all caseload.  During my supervision sessions, my supervisor and I will discuss your therapeutic needs.  Your confidentiality is maintained and no personal identification information is shared. I receive supervision on a regular basis to ensure my clients' safety, to maintain standards of professionalism and to work within my scope of practice. 

Other Medical or Psychological Treatment

If you are receiving care from any agency (GP, hospital, psychologist etc), permission must be received from that agency before any therapy can commence. The therapies I offer are not intended as primary healthcare but to work alongside the relationship which you have with your General Practitioner and are not to be used as a substitute for seeking medical attention.

Complaints Procedure

This is available on request from the National Hypnotherapy Society or the National Counselling Society.

Additional terms for working with persons under the age of 18 years

Where you are engaging me to work therapeutically with your child, you must accompany your child and remain in the room with us at all times.
